At Lombard Veterinary Hospital, we care for your pet like they were our own. Not only do we want to see your pet healthy, but happy, too! If you would like to send us some photos of your pet, please complete the form below and upload your favorite photo of your pet. 

Files must be less than 2 MB.
Allowed file types: gif jpg jpeg png.

I give Lombard Veterinary Hospital the right to publish pictures taken of my pet(s) during his/her/their stay at Lombard Veterinary Hospital. Images taken may be used as print, social media, online, and video based marketing materials.

I recognize that my participation is voluntary and that I will not receive financial compensation of any type associated with the taking or publication of these photographs or participation of marketing materials.

I waive the right to inspect or approve the finished product or copy and any use thereof.

I hereby release Lombard Veterinary Hospital and its employees involved in the creation or publication of marketing materials, from liability for any claims by me or any third party connection with my participation.
